The most recent offender comes from Netflix, and it's actually pretty difficult to spot. Redditor moofthestoof noticed that something was a little off with the cover photo for its horror flick The Babysitter.

If you're having trouble picturing it, try holding an object (not a knife) behind your back with your left hand, and notice the difference between your thumb, and the thumb in the photo.
